Promising aspects of ‘One-Stage Correction’ using frontalis muscle advancement flap in BPES syndrome
Synopsis :
A multistage approach is usually recommended for the management of blepharophimosis syndrome. We present a single-stage surgery in a 3-year-old female child, where initial correction of palpebral phimosis started with a lateral canthotomy and canthoplasty, followed by correction of epicanthus inversus by the Y-V plasty, and telecanthus by plication of the medial palpebral ligament. Finally, correction of the vertical component (bilateral ptosis) was addressed by brow suspension using frontalis muscle advancement flap. Single-stage correction via multiple surgical steps leads to satisfactory functional and aesthetic outcomes in all four classical components. The one-stage procedure has several advantages over multistage procedures such as a single surgical sitting, decreased hospitalization and recovery time, reduced exposure to general anesthesia, and cost-effectiveness, which effectively obviate the loss of follow-up seen with the multi-stage procedure in low socio-economic countries.
